To take the iPad home all forms (Acceptable Use Policy and Signature Form, 1:1 Insurance Form, GAFE Permission Form and Free and Reduced form if you wish) must be signed and turned in along with insurance paid (the forms are also available below for download). The cost for insuring the iPad for the year is $60.00. We will verify eligibility for Free/Reduced lunch and any refund ($60 Free, $30 Reduced) will be sent after October 1st.
